An acquired brain injury (ABI) presents a variety of challenges both to the affected person and those close to them. When the time comes for the client to leave hospital and return to their own home, or that of their family, we develop an individualised care plan, including input and guidance from appropriate healthcare professionals to enable a smooth transition into ongoing home based care
