Through our exceptional nurse led care we are able to provide highly specialist clinical care to clients in the comfort of their home. This mitigates risks and reduces hospital readmission rates for our clients.

At HFH, our complex care teams have helped many clients living across London return home from hospital and support them on their ongoing journey to recovery and rehabilitation.

30-year old Daniel was left paralysed from the neck down following a spinal injury. Our journey with Daniel began at Stanmore hospital where we coordinated with a range of healthcare professionals to get him back home in a safe and supported way. Due to the severity of Daniel’s injury he required us to provide 24-hour care with a 10 strong team to facilitate his complex care plan.

Daniel’s English was limited so we ensured that the care team we recruited could speak his own language. This allowed him to confidently communicate with our care teams and made sure his needs were being met in the best way possible.
