What Conditions do we Care for?

At HFH Healthcare, we specialise in providing high-quality complex care for a wide range of conditions affecting both adults and children. From complex medical needs to neurological disorders, our dedicated team are here to support you and your loved ones every step of the way.

Our services cover a wide variety of conditions, including but not limited to:

Spinal Cord Injuries:

Spinal cord injuries (SCIs) can result from a range of causes such as traumatic accidents, falls, sports-related injuries, medical conditions, and degenerative diseases.

The amount of support a person with a SCI needs depends on the extent of their injury. The more severe and the higher the injury, the greater the number of body systems that are affected. Our skilled carers are trained in the delivery and management of:

  • Autonomic dysreflexia
  • Airway suctioning
  • Bowel management
  • Catheter care
  • Complex medication administration
  • Complex moving and positioning
  • Cough assist
  • Enteral feeding
  • Invasive and non-invasive ventilation
  • Orthostatic hypotension
  • Standing frame/ use of splints & braces / passive range of motion
  • Tracheostomy care

Whether it be a complete SCI (involving a total loss of sensory and motor function below the level of injury) or an incomplete SCI (where some sensory and motor function remains below the level of injury), HFH understands the unique challenges that come with such injuries and offer personalised nurse-led care to enhance independence and quality of life for those living with spinal cord injuries.

Neurological Conditions (NCs)

These conditions encompass a wide range of disorders affecting the brain, spinal cord, and nerves throughout the body. NCs have many causes, including genetic factors, infections, trauma, and autoimmune disorders. They are very common, with 1 in 6 people in the UK currently living with such a condition.

An example of some of the neurological conditions we care for include:

  • Huntington’s Disease
  • Epidermolysis Bullosa
  • Epilepsy
  • Parkinson’s Disease
  • Multiple Sclerosis
  • Cerebral Palsy
  • Motor Neurone Disease
  • Stroke
  • Muscular dystrophy

Our complex care packages address the diverse needs of individuals with neurological conditions. These include:

Physical Assistance: We provide support with daily activities like dressing, bathing, eating, and safe mobility for those facing challenges with coordination or muscle control.

Medication Management: Our team ensures safe administration, dosage, and monitoring of complex medication regimens crucial for symptom management and disease progression control.

Emotional and Psychological Support: Recognising the emotional challenges of neurological conditions, we provide emotional support to improve our client’s overall outlook and well-being.

Rehabilitation and Therapy: Specialised rehabilitation and therapy can improve or manage many neurological conditions. Our complex carers support clients in achieving their rehabilitation goals throughout their care journey.

Brain Injury Care:

At HFH Healthcare, we provide specialised support for individuals who have acquired brain injuries, focusing on rehabilitation, and promoting cognitive and physical well-being.

An example of some of the brain injury conditions we care for include:

  • Head trauma
  • Birth trauma
  • Tumour
  • Stroke
  • Brain haemorrhage
  • Infection: encephalitis or meningitis
  • Hydrocephalus

The severity of these injuries can vary widely from person to person. Some may experience improvement over time, while others may face permanent impairment.

At HFH, we offer holistic care for individuals with acquired brain injuries, addressing their physical and emotional needs. Our excellent carers assist with daily living, medication management, communication needs, emotional support and social interaction to enhance quality of life and happiness.

Airway & Respiratory Care:

HFH Healthcare understands that exemplary airway and respiratory care plays a vital role in supporting someone with complex health needs.

We support clients with:

  • Tracheostomy care
  • Invasive and non-invasive ventilation
  • Airway suctioning
  • Cough assist
  • Oxygen therapy and monitoring

We conduct thorough respiratory assessments and monitoring to ensure the safety and well-being of people in our care. Our professionals are skilled in monitoring vital signs, oxygen saturation levels, and respiratory function, allowing for early detection, escalation and management of any potential issues.

As experts in the care of individuals requiring ventilator support, airway suctioning, tracheostomy care and ongoing monitoring, we are committed to making our clients feel as safe and comfortable as possible throughout every stage of their care. Our Clinical Team work alongside community long-term ventilation teams to ensure best practices and high standards of care are always provided.

Learning Disability & Autism Care:

Learning disabilities (LDs) are different for everyone, with the degree of disability varying from person to person. They can be classed as mild, moderate, severe, or profound.

Common learning disabilities we care for include:

  • Global Developmental Delay
  • Profound and Multiple Learning Disability (PMLD)
  • Autism Spectrum Disorder (ASD)

Those with LDs may find it tough to process information, learn new skills, perform daily tasks and may also experience communication difficulties. As conditions present differently in each person, care must be personalised for every client.

At HFH, we provide high-quality complex healthcare to adults and children with learning disabilities who also require support with a continuing complex health need.

Our care plans are designed to meet the needs of the individual, including the emotional, physical, and social needs of each of our clients. We listen to and work with our clients to understand their goals and aspirations. We make sure to carefully match them with the best carers for their specific requirements, encouraging them to build a bond and trusted relationship. All appropriate considerations are made to ensure we provide the best quality care for our clients.

Living with a learning disability brings countless challenges to individuals and their families in their daily lives. Our service makes it possible for them to receive safe and quality care in their home should they have additional complex health needs.

Genetic and other conditions:

Genetic conditions affect many individuals living in the community. Such conditions include Down syndrome, Cystic fibrosis and Sickle cell anaemia, among many others.

Due to our extensive experience delivering safe and effective care for those with complex health needs, we also support individuals living with:

  • Chiari malformation
  • Diabetes
  • Spina bifida
  • Scoliosis
  • Turner syndrome
  • X-linked myotubular myopathy
  • Pfeiffer’s syndrome
  • Spinal muscular atrophy

HFH tailor every care plan to each client’s individual needs. Every package is overseen by experienced Nurse Case Managers who work together with other medical professionals to ensure the client’s needs and health requirements are fully considered throughout the entire process.
